Cornerstone also addresses the needs of the legal, insurance, and employment systems, as well as families and caregivers. Additionally we complement the services of other allied health professionals.

In-Home Functional Assessments

An in-home assessment is an individualized assessment of a person’s ability to carry out their day-to-day tasks. This can include self care (dressing, bathing, moving about), home management (preparing meals, performing household chores), and leisure activities.

A Cornerstone occupational therapist will consider your physical abilities and thinking skills, and the environment in which you live, keeping in mind your goals and desired activities. You will be provided with recommendations for ways to improve your safety and efficiency in these everyday tasks. Suggestions may include specialized equipment, adapted ways of performing tasks, or changes to the environment. We are able to complete basic seating and mobility assessments and recommendations.

Mental Health Assessment and Intervention

Cornerstone occupational therapists have significant experience in the area of mental health rehabilitation. Using objective assessments, in-person interviews and observation, we assess the functional, daily life consequences of mental health conditions such as depression and anxiety.

Intervention meets clients where they are in terms of recovery, and focuses on the use of daily activities and routines to promote positive mental health and to return to desired activities.

Brain Injury Assessment and Intervention

Cornerstone occupational therapists have extensive experience in the area of brain injury rehabilitation. Using objective assessments, in-person interview and observation, we assess the functional, daily life consequences of brain injury. Intervention helps individuals and their families to better understand the cognitive and behavioural changes which can occur following brain injury, provides recommendations to help maximize recovery, and assists with developing strategies to compensate for cognitive difficulties at home, at work, or in the community.

Life Care Planning

A Life Care Plan is a medical legal document describing the costs of future care and services required for an individual, as a consequence of their accident or injuries.

A Life Care Plan completed by Cornerstone assists in determining the impact of a disability on all aspects of a client’s life, and utilizes documented professional guidelines and current best practice standards to prepare a comprehensive, defensible report. A Life Care Plan can be prepared for the plaintiff or the defense, using the same objective methodology, and helps to establish the monetary value of an injury, thus assisting with establishing a settlement.

We are also able to provide a critical review of a life care plan prepared by another provider.

Ergonomics

Ergonomics

Our therapists have experience in office and industrial ergonomic assessments, and has been a member of the Association of Canadian Ergonomists since 2008. Ergonomic assessments and intervention can help to decrease risk of workplace musculoskeletal disorders. This can be a preventative measure, or as part of a return to work program after an employee has had an injury. Ergonomic education for employees can also increase awareness of work behaviors that may contribute to injuries, thus giving employees increased responsibility for their own health in the workplace.

We provide a wide variety of services to meet your ergonomic needs and budget. These include:

  • Comprehensive assessments with detailed reports
  • Comprehensive assessments with brief reports (recommendations only)
  • Brief preventative assessments
  • Group education
  • Train the Trainer education

Gardening

Health and Wellness Programs

We offer a number of individual and group programs and sessions, designed to meet your needs. We can help you determine your specific learning objectives, and identify goals that are practical and meaningful. Our sessions incorporate active participation and hands-on experiences for enhanced learning. Sessions are customized for participants and can be carried out in the home, school, workplace, or community setting. This is often a cost effective approach to address issues such as injury prevention.

Job Demands Analysis

A Job Demands Analysis (JDA) provides an objective, detailed description and classification of the physical, social and cognitive requirements of essential job tasks so that the employer may match workers to jobs that they are able to competently and safely perform. This can be used in pre-placement, return to work and for injury prevention.

Disability Case Management

Disability Case Management (DCM) facilitates the employment of persons with a disability though coordinated efforts by addressing individual needs, the work environment, company needs and legal (duty to accommodate) responsibilities. A Cornerstone occupational therapist works directly with all players (i.e. family physician, employers, unions, work place supervisors, and other allied health professionals) to direct a successful and timely return to work plan.

Hand function Assessment

Vocational Consultation:
Return to Work

Involves working closely with the employee, employer and other stakeholders, to negotiate and facilitate a successful return to work (RTW) after an absence due to illness or injury. This may involve a full assessment of the client’s abilities and limitations, and a job demands analysis to determine job match, and then to make recommendations for accommodations. This can help employers meet their responsibilities under the Duty to Accommodate legislation.

Vocational Directions Assessment: Transferable Skills

Vocational Directions Assessment (VDA) is used to identify realistic, attainable and cost-effective vocational options for individuals who require assistance in changing their career path.

The process includes an in-person interview with the client to obtain vocational history, non-paid work experiences, educational/training background, transferable skills, special skills, work successes / challenges and areas of interest. It may also include an assessment of current physical, cognitive and social functioning and a preliminary Labour Market Survey.

Lift Overhead

Functional Capacity Evaluation: FCEs

Functional capacity evaluations (FCEs) are objective one or two day comprehensive assessments designed to determine a client’s physical abilities. Our FCEs are customized to answer the questions of the funder, which may include questions about a person’s ability to meet the demands of their job, home or leisure activities. It includes components such as positional tolerances, range of motion, strength, hand coordination, balance, lifting, carrying, pushing/pulling, sitting, standing, walking, climbing, kneeling, squatting, reaching and bending. We provide a safe environment for the client to demonstrate their full physical capacity, while measures are in place to ensure the client is giving their best effort.

Functional Cognitive Assessment 

Functional Cognitive Assessment is a comprehensive evaluation of how a client’s thinking skills affect their daily activities.

This includes in-home assessment of the client’s functional cognitive abilities, including:

  • Narrative assessment and observation of the client in their home environment
  • Review of background/medical information if available
  • Combination of standardized and non-standardized assessments, customized to meet the specific needs of the referral source
  • Detailed report describing the client’s functional cognitive abilities and limitations
  • Miscellaneous including travel and follow up with the client and insurer as needed.

If the file review and/or travel are extensive, additional time may be billed.

Office based assessment may also be arranged if required.

Autism Support (Consultation and Intervention)

Cornerstone occupational therapists have extensive experience working with individuals with autism, and their families. Our therapists can provide coaching and consultation regarding behaviours, daily routines, and function in the home and community. Areas of support can include daily activities such as eating, toileting, hygiene, and sleep, behavioural support, as well as support for development of play, social communication and self-regulation skills. Our services are tailored to the needs of the individual and/or the family, and our goal is to support families to support their child.

Our services can be provided virtually, or within your home, in certain geographic areas.

We aim to provide cost effective services for our clients. Many of our services are covered by employer or personal insurance plans.
